The one problem with being too reliant on the ASP or SaaS model is what a friend of mine used to refer to as the "idiot on the backhoe". Can you effectively operate if your telecom capability suddenly is lost? In healthcare this is particularly critical when you start talking remote hosting EMR's and such. I worked briefly with VHA and although they have been trying for years to create an internal ASP type model they realized that certain IT capabilities had to remain at the local hospitals if only as backup. While the ASP/SasS model will be very tempting for smaller practices that have no existing internal IT support, they will have to look very carefully at the possible liabilities.
